To complete your application you need to do the following steps:

• 1. Create a SIPSI-account. https://www.sipsi.travail.gouv.fr (please, only create an account at this moment) You may use this aid. Do you want us to create your SIPSI-account, please visit our page below: www.transportinnood.nl/sipsi/help/en/
• 2. Fill in this driverform completely. You can also fill in your login details on this form. We need this file digitally and in this format.
• 3. Please fill in the Applicationform digital (Use Adobe Acrobat).
• 4. Please scan your copy of the registration at the Chamber of Commerce.
• 5. A driver must have a valid A1 form with them (How to apply for one, you can read at this link: SVB_A1.pdf, Dutch only) Make sure you have a copy of the A1 form we might, as a representative, ask for it. So don't send us the A1-form yet. You can only apply for this A1 document to the SVB for workers in the Netherlands. For foreign offices you can find it here.
